fa مقایسه شدت فراوانی اختلال وسواسی– جبری در کودکان مبتلا و غیر مبتلا به بیماری مزمن کلیه Comparison of obsessive-compulsive disorder in Children with and without CKD in Amirkabir hospital of Arak تخصصي Special پژوهشي Research مقدمه : بیماری مزمن کلیوی یک بیماری طبی مزمن و شایع دوران کودکی که یکی از مشکلات آن اختلالات روحی و روانی است. هدف از مطالعه حاضر، مقایسه شدت فراوانی اختلال وسواسی– جبری در کودکان مبتلا و غیر مبتلا به بیماری مزمن کلیه، می باشد. روش کار : این مطالعه به روش مورد – شاهدی برروی160   کودک 7-17 سال مراجعه کننده به بیمارستان امیرکبیر اراک، انجام گرفت. 80  کودک مبتلا به درجات اولیه نارسایی مزمن کلیوی مرحله(1 تا 3) به عنوان گروه مورد و   80 کودک سالم بعنوان گروه شاهد، به شکل تصادفی ساده وارد مطالعه شدند.پرسشنامه اطلاعات جمعیت شناختی و پرسشنامه اختلال وسواسی-جبری تکمیل شد. یافته ها: میانگین نمرات چک کردن/شک داشتن(در گروه مورد 2/54±3/52 و در گروه شاهد:2/32±2/5، 0/007p=) و مرتب کردن (در گروه مورد:86/1±59/2 و در گروه شاهد:2/56±1/5، 0/005p=)، در گروه کودکان مبتلا به نارسایی مزمن کلیوی به طور  معناداری بیشتر از گروه کودکان سالم بود. همچنین میانگین جمع نمرات پرسشنامه اختلال وسواسی- جبری در گروه کودکان بیماری مزمن کلیه با 7/69±15/32 به طور معناداری بیشتر از گروه کودکان سالم با 2/54±11/12 بود (0/021p=). میان مدت زمان ابتلا به بیماری مزمن کلیه و شک داشتن/چک کردن (0/004p=، 0/4r =)، افکار وسواسی (0/06p=، 0/02r =)، شستشو (0/031p=، 0/8r =)، مرتب کردن (0/001p=، 0/2r =) و  مجموع نمرات پرسشنامه OCI-CV (0/04p=، 0/4r =) همبستگی معناداری وجود داشت. نتیجه گیری : اختلال وسواسی- جبری در کودکان مبتلا به نارسایی مزمن کلیوی به شکل معناداری بیشتر از کودکان سالم است. از اینرو ، توصیه به انجام مداخلات روانپزشکی در کودکان مبتلا به نارسایی مزمن کلیوی مفید به نظر می رسد، با این حال انجام مطالعات بیشتر در این زمینه جهت حصول نتیجه گیری قطعی تر توصیه می گردد. Introduction: That one of the problems is mental disorders. The aim of this study was to investigate obsessive-compulsive disorder in Children with and without CKD in Amirkabir hospital of Arak. Method: 80, 7-17-year-old children with CKD and 80 healthy children without CKD were included in this case – control study as case and control groups, respectively. The participants were selected from those children who were referred to the pediatric clinic of Amir Kabir Hospital of Arak (Iran) in the form of simple probability and based on inclusion and exclusion criteria. Questionnaire on demographic and clinical information and the OCI-CV (for obsessive-compulsive disorder) were completed. Data were analyzed by t-test test in SPSS18. Results: The mean doubting/checking (p=0.007) and ordering (p=0.005) in children with CKD was significantly higher than healthy children. Average total OCI-CV scores in the case group with 15.32±7.69 was significantly more than the control group with 11.12±2.54 (p=0.021). There was significant correlation between duration of CKD and doubting/checking (p=0.004), obsession (p=0.06), washing (p=0.031), ordering (p=0.001) and all OCI-CV point (p=0.04).
