fa تببین تجربیات پرستار انکولوژی از مراقبت از کودکان مبتلا به سرطان در شرف احتضار: مطالعه موردی مبتنی بر روایت های بالینی تخصصي Special گزارش مورد case report <div style="text-align: justify;"><span style="font-size:18px;"><strong><span style="font-family: b nazanin;">مقدمه</span></strong><span style="font-family: b nazanin;">: مراقبت از بیمار در حال احتضار برای هر فردی تنش زا است. هنگامی که بیمار کودک باشد و علت فوت او بیماری سرطان باشد این تنش افزایش می یابد. مراقبت ازکودک در حال احتضار امری بسیار دشوار است. بازاندیشی بر رویدادها به پرستاران کمک خواهد کرد تا راهکاریی مفید برای انطباق با بیماری را شناسایی نمایند و بهتر بتواند به بیمار خود خدمت ارائه دهند. هدف از این مطالعه تبیین تجربیات پرستار بخش انکولوژی کودکان از مراقبت در لحظات بحرانی و پایانی زندگی کودکان مبتلا به سرطان در شرف مرگ است.</span><br> <strong><span style="font-family: b nazanin;">روش:</span></strong><span style="font-family: b nazanin;"> این مطالعه یک گزارش مورد با رویکرد کیفی است که برای گردآوری داده از روایت های بالینی پرستار با تجربه در بخش انکولوژی اطفال استفاده شد. 30 روایت بالینی مکتوب بر اساس روش تحلیل محتوای کیفی مرسوم</span><span style="font-family: b nazanin;">، </span><span style="font-family: b nazanin;">کدگذاری، طبقه بندی و مضامین استخراج شدند. تحلیل داده ها با تاکید بر آنالیز متن در خصوص &quot;انگیزه مراقبت از کودک در شرف مرگ، چالش های مراقبت از کودک در شرف مرگ، تاثیرگذاری از مرگ کودکان، عوامل موثر بر ابقا در بخش انکولوژی علی رغم مواجه مکرر با مرگ کودکان&quot; بوده است. </span><br> <strong><span style="font-family: b nazanin;">یافته ها</span></strong><span style="font-family: b nazanin;">: از تحلیل داده ها چهار مضمون اصلی &quot;برآورده ساختن آرزوی کودکان&quot;، &quot;تجربیات باور نکردنی در لحظات پایانی زندگی&quot;، &quot;تسلیم در مقابل خواسته خدا&quot; ، &quot;تصمیم گیری مبتنی بر باورها&quot; استخراج شد. داده ها حاکی از آن است که برای ایجاد آرامش به هنگام مرگ، به والدین توصیه می شد که آخرین ساعات عمر را با کودک خود در خارج از بیمارستان سپری کنند. تعلق عاطفی پرستار به کودکان بیمار و شاهد مرگ آنان بودن، تجربه بسیار دردناکی توصیف شد که با شرکت در مراسم ختم کودکان، از فشار روانی کاسته می شد و سبب التیام روحی خانواده می گردید.</span><br> <strong><span style="font-family: b nazanin;">نتیجه گیری:</span></strong> <span style="font-family: b nazanin;">اگرچه ارائه مراقبت برای کودک مبتلا به سرطان از نظر عاطفی برای پرستار رنج آور و تنش زا است، در صورت فراهم آوردن شرایط مرگ توام با آرامش، با رضایت خاطر و وجدان آرام برای پرستار و خانواده خواهد بود.</span></span></div> <div style="text-align: justify;"><strong>Introduction </strong>: Care of dying patients is stressful for everyone. When the patient is a child and her cause of death is cancer, the tension rises. Care of dying child is very difficult. Reflection on the events will help nurses identify useful strategies for coping with the disease and provide better service to their patients. The aim of this study was to explore pediatric oncology nurse&rsquo;s experiences of caring in the end of life and critical moments of dying children with cancer.<br> <strong>Methods</strong>: This study is a case report with a qualitative approach. The data collected through documented clinical narrations by an expert nurse in pediatric oncology. 30 written clinical narratives were coded, classified and themes extracted using conventional qualitative content analysis<span dir="RTL">.</span> The texts were analyzed with an emphasis on finding themes about &ldquo;motivation of caring of dying children, challenges of caring of dying patients, factors&nbsp;that&nbsp;affect oncology nurse&nbsp;outcomes, and factors to continue working in oncology unit&rdquo;.<br> <strong>Results</strong>: Four main themes were emerged from data analysis &quot;Fulfilling the dreams of children<span dir="RTL">&quot;</span>, &quot;surrender to God&#39;s will&quot;, &quot;unbelievable life&#39;s final minutes&quot;, and &quot;faith-based decisions&quot;. The data revealed that to create peace for dying, parents were advised to spend the last hours of child life with their child outside of hospital environment. The emotional attachment of the nurse to ill children and be witness of their deaths were described very painful experiences.
