fa شاخص های حرکتی دهانی شیرخواران طبیعی 4 تا 9 ماهه ایرانی: یک گزارش کوتاه Oral Motor Indexes in Iranian 4- to 9-month-old infants: A short report تخصصي Special پژوهشي Research <div style="text-align: justify;">مقدمه  امروزه بررسی شاخصهای حرکتی دهان در شیرخواران، به تیم درمان کمک می‌کند تا شیرخوارانی که در این شاخصها دچار تاخیر هستند را زودتر شناسایی کنند و  از ترتیب ظهور این شاخصها در شیرخواران طبیعی، به عنوان معیاری برای درمان شیرخواران با تاخیر حرکتی استفاده کنند. هدف از مطالعه حاضر، تعیین زمان ظهور شاخصهای حرکتی باز کردن دهان در هنگام چسبیدن قاشق به لب ها، حرکــت آرام زبان به جلو و عقب در هنگام ورود غذا به دهان و حرکت به طرفیــــــن  بردن زبــــــــــان و  فک در نوزادان طبیعی ایرانی 4 تا 9 ماهه شهر شیراز است. روش در این مطالعه مقطعی، در سال 1397 ، با استفاده از نمونه‌گیری خوشه‌ای،250 شیرخوار 9-4 ماهه طبیعی، از 7 مرکز بهداشت انتخاب شدند. پس از تایید رشد طبیعی این شیرخواران با استفاده از(ASQ)  The Ages & Stages Questionnaires یک کارشناس گفتاردرمانی در محیط مرکز بهداشت در ساعت تغذیه کودک، الگوهای حرکتی فک، زبان و لبها را در خوردن نیمه جامدها و جامدها در 6 گروه سنی 4، 5، 6، 7، 8، و 9 ماهه ، با استفاده از چک لیست محقق ساز الگوهای حرکتی حرکات فک و زبان، ارزیابی و تحلیل کرد. یافته ها نسبت روایی محتوایی چک لیست ارزیابی حرکات دهانی 0/99 بود. تمام شیرخواران 9-4 ماه، دو شاخص حرکتی باز کردن دهان در هنگام چسبیدن قاشق به لب ها، حرکــت آرام زبان به جلو وعقب در هنگام ورود غذا به دهان را نشان دادند. در بررسی حرکت طرفی زبان، 13/6% کودکان 4 ماهه فقط حرکت زبان به طرف راست را نشان دادند و 40/9% چرخش زبان به طرف راست و چپ را نشان دادند. و به ترتیب شاخص چرخش زبان به طرف راست و چپ در 5، 6، 7 ، 8 ، 9 ماهه‌ها، 73/5%، 59% ، 80/8%، 91/4% ، 93/3% بود. حرکت به طرفیــــــن  بردن زبــــــــــان در سن 5 ماهگی و حرکات جانبی فک، در 8 ماهگی در شیرخواران ایرانی دیده‌‌شد. نتیجه گیری به نظر می‌رسد که سن ظهور برخی از شاخص‌های مذکور در شیرخواران ایرانی مشابه سایر نژادها و در برخی  شاخصهای دیگر، مانند حرکات آرام جلو به عقب زبان هنگام ورود ماده غذایی به دهان، زودتر از نژادهای دیگر ظهور پیدا کرده‌است. ضروری است که مطالعات تعیین شاخص‌های حرکتی، در جمعیتهای مختلف بطور جداگانه صورت گیرد، تا اطلاعات دقیق‌تری جهت تعیین تاخیرهای حرکتی ناحیه دهان، در اختیار درمانگران قرار گیرد.</div> <div style="text-align: justify;">Introduction: Examination of oral motor indices in infants helps treatment team members identify infants who are delayed in these indices earlier and use the order of occurrence of these indices in normal infants as a criterion for treating infants with motor delay. Literature shows that oral movement indices are different in different races, cultures, and languages. The aim of this study was to determine the time of onset of movement indicators of opening the mouth when the spoon sticks to the lips, gently moving the tongue back and forth when food enters the mouth, and lateral movement of the tongue and jaw in Iranian normal infants in Shiraz. Method: In the current study, in 2018, by using cluster sampling, 250 infants 4-9 months were selected from 7 public health centers in Shiraz.  After conferring the participants ‘normal development, by using The Ages & Stages Questionnaires(ASQ), a speech therapist assessed and analyzed the lips, tongue, and jaws movements during feeding of semi-solid and solid food by using researcher developing Questionnaire in public health centers in 6 age groups include 4,5,6,7,8, and 9 months.   Results: The oral movements checklist content validity ratio (CVR) were 0/99. The oral movements, opening the mouth when the spoon sticks to the lips, moving the tongue gently forward and backward when food enters the mouth were observed in all participants 4-9 months. In evaluating lateral movement of the tongue, 13.6% of infants with 4 months showed only the tongue movement to the right, and 40.9% showed the tongue movement to the right and left. the tongue movement to the right and left in infants with 5, 6, 7, 8, and 9 months were observed in 73.5%, 59%, 80.8%, 91.4% and 93.3% respectively.  Moving to the side of the tongue and lateral jaw movements are seen in Iranian infants at the age of 5 and 8 months, respectively.
