Introduction: Childhood cancer causes physical and mental traumatic situations. This can create problems for the growth and development of self-perception, especially in children. This study was conducted to explain the experiences of children with cancer regarding their self-perception. Method: This qualitative study was a content analysis study with the participation of 21 children, family members, and healthcare professionals, who were selected using a purposive method. Data were collected through individual and paired interviews, focus group discussions, and field notes. Data analysis was performed using content analysis and simultaneously with data collection based on the five steps of Elo and Kingas' approach. The accuracy and robustness of the research were ensured using the criteria of Guba and Lincoln. Findings: A total of 400 codes were obtained from data analysis, and by continuously comparing the codes, the main themes of unstable self-perception and the themes of distorted body image (physical self) and shaky personal self (personal self) were abstracted. Conclusion: The body image and identity of children and adolescents with cancer are easily affected by physical changes and experiences caused by the disease and its treatment. The child's instability in self-evaluation will lead to an unstable self-perception and unclear identity, which can significantly affect their well-being. Paying attention to the child's feelings, helping him/her understand the changes, reinforcing strengths and positive thinking, setting achievable short-term goals, and providing spiritual support can strengthen self-concept and positive body image in these childre
