Effectiveness of Family-Based Cognitive-Behavioral Therapy on Worry, Avoidance, and Severity of Symptoms in Children with Obsessive-Compulsive Disorder

Introduction : Obsessive-compulsive disorder in children is a form of anxiety that is accompanied by worry and avoidance thoughts, and it is important to equip parents and children for effective treatment. It is crucial to equip both parents and children for effective treatment. Therefore aim of the present study was to assess the effectiveness of family-centered cognitive-behavioral therapy on anxiety, avoidance, and the severity of symptoms in children with obsessive-compulsive disorder.
Method: The research method was semi-experimental. The statistical population consisted of children with obsessive-compulsive disorder from schools in Isfahan in 2024, along with their mothers. From this population, 36 mother-child pairs were purposefully selected and randomly assigned to experimental and control groups using random numbers. A pre-test was conducted. The experimental group received 12 sessions of family-centered cognitive-behavioral therapy once a week, while the control group remained on a waiting list. After the completion of the therapy sessions and 45 days later, both groups were re-evaluated. The research tools included the Pennsylvania State University Worry Questionnaire (1997), the Children's Avoidance Questionnaire by Whiteside (2013), and the Children's Obsessive-Compulsive Inventory by Foa (2010). Data were analyzed using repeated measures ANOVA with SPSS26 software.
Result: The results showed that family-based cognitive-behavioral therapy had a significant effect on children's anxiety, avoidance, and obsessive-compulsive symptoms (p ≤ 0.05), and this effectiveness persisted during the follow-up phase. Additionally, there was no significant difference between the post-test and follow-up results (p≥0.05). Furthermore, this effectiveness was maintained during the follow-up phase.
Conclusion: Given the effectiveness of family-centered cognitive-behavioral therapy in modifying avoidance behaviors, obsessive-compulsive symptoms, and worry in children, it is recommended that therapists consider this treatment for families and children exhibiting obsessive-compulsive symptoms.
